Diabetes Education & Prevention

Six Montgomery County regional hospitals tackling diabetes to help those at risk of developing diabetes and those who have diabetes.

Diabetes Prevention Program
​(DPP)
Program Designed for: At risk of developing diabetes.

Program Duration: 16 weekly core sessions, followed by bi-monthly/monthly maintenance sessions.

Program Eligibility:
  • Must be 18 years or older
  • All program participants must have a body mass index (BMI) of ≥ 25kg/m2 (≥ 23kg/m2 if Asian American)
  • Must have a recent blood test indicating prediabetes (within the past year)

Program Overview: A free lifestyle change program that offers individuals the support they need to make sustainable lifestyle changes to improve their health and prevent the onset of diabetes.
  • Dedicated lifestyle coach leading a CDC-approved curriculum
  • Nutritional and physical activity guidance
Diabetes Self-Management
Education & Support
​(DSMES/DSMT)
Program Designed for: Individuals with diabetes.​

Program Duration: 10 hours of curriculum during the first year - includes both one-on-one and group sessions.
  • Up to 2 hours of ongoing maintenance sessions per year.

Program Eligibility:
  • Must have a diabetes diagnosis from your provider and:
    • Fasting blood glucose of ≥ 126 mg/dl on two different occasions
    • 2 hour post-glucose challenge ≥ 200 mg/dl on two different occasions
    • Random glucose test over 200 mg/dl for a person with symptoms of uncontrolled diabetes

​Program Overview: Focused on attaining the knowledge, skills, and support needed to work with one's physician and successfully "self-manage" their diabetes on a daily basis.
  • Diet & Nutrition guidance
  • Blood glucose self-monitoring
  • Medication adherence/education
  • Risk-reduction of complications
  • Coping strategies, problem-solving
  • Support from a registered dietician
