A collaboration among Montgomery County's hospitals, Nexus Montgomery works with community partners to promote health, reduce hospitalization utilization and manage total cost of care of our share community in ways that no single hospital could achieve on its own.

Nexus Montgomery will establish itself as a center of innovation and a model for collaboration by jointly managing a dynamic portfolio of programs that is developed in response to our evolving economic, social, and healthcare environments, implemented efficiently, and continuously evaluated for effectiveness and stewardship of resources

​What is Nexus Montgomery?

The six hospitals operating in Montgomery County, Maryland have joined forces with a network of community-based organizations to form the Nexus Montgomery Regional Partnership. Nexus Montgomery is a hospital-led collaborative that aims to reduce avoidable or unnecessary hospital use - including readmissions, by connecting people to timely and appropriate community-based care and support services. ​Nexus Montgomery manages a portfolio of programs operated by the Primary Care Coalition (PCC). PCC has expertise in operationalizing multi-partner initiatives and serves as the professional staff leading implementation of Nexus Montgomery programs. ​​

How does Nexus Montgomery keep the community healthy?

Patients often receive care from more than one hospital. Nexus Montgomery formed because hospitals can better serve the community by working together. Our programs improve the health of the community by supporting coordinated services that are tailored to the needs of individuals. Our focus is on addressing the medical and social factors that contribute to health and wellbeing.

Nexus Montgomery invests in programs and initiatives that will improve the health of people who are at high risk of an adverse health event, or who have complex needs. By keeping people healthy and connecting them to appropriate community-based support, Nexus Montgomery initiatives reduce avoidable hospital use, reduce the total cost of hospital care, and generate a positive financial return for the community and hospitals.
